How does home health care begin?
Home health begins with a referral from your doctor and it usually begins within 24-28 hours depending on patient’s medical needs. On call services are available 24 hours, 7 days a week.
What are the criteria for eligibility?
- Patient has to be homebound
- Patient has to be under care of a licensed physician
- Patient has to have a skill need.
What is the coverage for services?
Home health is covered under Medicare, Medicaid and most private insurance plans, HMO’s and other managed care plans.
Can we keep our own doctor?
Yes! You can keep your own doctor. SmartCare Home Health Services has a health care professional team that will coordinate a care plan with your physician. We remain on-call 24 hours every day of the year.
After finalizing our contract, a SmartCare caregiver will be assigned to coordinate your care at home. He or she will work with your physician to make sure all your needs are met. Your caregiver will make sure you receive specialized help, from emotional support to physical therapy, to wound care.
